Eligibility: |
| 1. |
Be a registered professional
nurse and a member of MASN for at least five years and NASN for the current year and the preceding two
years. The preceding two years must be continuous without lapse in membership. |
| 2. |
Nominee must have five years
experience as a school nurse and be currently employed as a full-time school nurse. If the only school
nurse position in a community involves fewer hours than usual, it will be considered full time for the
purpose of this eligibility criteria. More than 50 percent of nominee's time must be spent in direct care. |
| 3. |
Nominee may not be on the NASN BOD
or an officer of NASN at the time of the nomination. |
| 4. |
Evidence of excellence in school
nursing practice must be based on Scope and Standards of Professional School Nursing Practice
(copyright 2005, National Association of School Nurses and American Nurses Association). Please refer
to NASN Operating Guidelines Criteria for Selection and use the form found at bottom of this web page.

for NASN SNOY to be selected. |

MASN guidelines for SNOY will be
changed as needed to reflect current NASN guidelines. |

Procedure for Submission of Application: |
| 1. |
Information must be submitted
in an 8 ½" X 11" flat folder. (No ring binders). |
| 2. |
Three nomination folders
(the original and two copies) should be submitted by the nominee's representative, postmarked
no later than January 15, to the Awards Committee chair. |
| 3. |
Include in the nomination folder: |
|
a. |
MASN nomination form signed by the
district president. |
|
b. |
Letter from district organization
stating support and rational for nomination signed by district president. |
|
c. |
Curriculum vitae signed by the nominee |
|
d. |
Narrative describing the nominee's
contribution in each of the categories named in the Criteria for Selection and should follow the categories
and sub-areas exactly in outline form. Narrative must be signed by nominee. (See forms links at the bottom
of this webpage for more details). |

Maximum of six letters, each limited to one
page, no reductions permitted. |

Letters may be from school nurse colleagues,
administrators, supervisors, teachers, parents, students and others. |

Letters should describe specific issues
or topics related to nominee's qualifications for the award. Use Scope and Standards of Professional
School Nursing Practice (copyright 2005, National Association of School Nurses and American
Nurses Association) as a reference. |

Letters should directly reflect areas of rating
that writer wishes to emphasize. |

º |
Local and state acronyms must be spelled
out when used for the first time. |

One photo which will not be provided to
reviewers or judges but kept by the Awards Committee chair and may be used to recognize (honor) MASN
SNOY applicants. No photos should be used in the application. |
| 4. |
The total folder is not to exceed 20 pages
(20 one-sided or 10 two-sided). This does not include the cover or the application page itself. |

Selection Procedure: |
| 1. |
Nomination folder must be postmarked
no later than January 15. |
| 2. |
Only one SNOY award is granted anually. |
| 3. |
Awards Committee chair verifies MASN and NASN
membership and other required eligibility information requested on nomination form. |
| 4. |
If nominee does not meet eligibility or
submission criteria, nomination folder will be returned prior to scoring with a letter of explanation
from the Awards Committee chair. |
| 5. |
Awards Committee reviews and scores
the nomination folders. The Awards Committee chair will arrange for a neutral person or persons to
be involved in award selection if a member of the committee has a conflict of interest (i.e., the
committee members are from the same district as the nominee). |
| 6. |
Tally sheets will be returned to
Awards Committee chair by February 1. The Awards Committee chair tallies the sheets to determine the
winner. |
| 7. |
The Awards Committee chair notifies
committee members of the winner. |
|
Award Process |
| 1. |
The Awards Committee chair will
notify the presidentof the MASN SNOY recepient by February 15. |
| 2. |
President will telephone the
selected nominee by March 1 and will mail a letter to the recipient and all other nominees by
March 5. |
| 3. |
President will notify the recipient's
MASN district president and school administrator by March 1. |
| 4. |
Awards Committee will submit
MASN SNOY nominee folder to NASN according to NASN guidelines, postmarked no later than October 1.

| 5. |
The Awards Committee will purchase
a plaque and coordinate the purchase of a corsage for the award recipient with the MASN district
conference host. The Awards Committee chair will present the plaque at the annual meeting. |
| 6. |
The honoree and one guest will
have luncheon or banquet dinner cost paid for by MASN. |
